Case History Form: This is a detailed Google Form designed to gather a thorough medical and social history. By completing it, you give us valuable insight to tailor our services to your child’s unique needs.

Signature Forms: A set of Adobe forms that require signatures, covering essential agreements and consents.

The entire process will take more than 30 minutes, so we recommend setting aside at least one hour to ensure you can provide all the necessary information. Once submitted, your paperwork will be reviewed by our new client coordinator, and we will confirm completion prior to your first appointment.
